Case Acceptance Without Pressure

The 95–5 Rule: Where Case Acceptance Is Won

One of the most impactful concepts introduced was the 95–5 Rule.

  • 95% of the conversation should focus on the condition—what’s happening, why it matters, and where it’s heading.
  • 5% of the conversation is reserved for the solution.

Most practices reverse this—jumping straight to crowns, implants, or procedures before patients understand the “why.” The result? Confusion, hesitation, and cancellations.

Instead, Eric demonstrated how vivid language, storytelling, and imagery help patients clearly visualize what’s happening in their mouths—long before treatment is ever discussed.

A cracked tooth becomes a log splitting under a wedge. Decay becomes a worm burrowing toward the core of an apple. These stories stick—and patients can explain them later to spouses and family members.


Overhear Psychology and Implication Questions

Rather than telling patients directly what they need, the system uses Overhear Psychology—doctor-to-assistant dialogue that patients naturally listen to and absorb. This creates co‑diagnosis instead of confrontation.

Layered into this are implication questions, which gently guide patients to recognize consequences on their own:

  • “Has this tooth started to bother you yet?”
  • “Have you ever had a toothache before?”
  • “Do you know anyone who lost teeth due to gum disease?”

These questions don’t scare patients—they clarify reality. The focus stays on outcomes like toothache and tooth loss, not procedures.


People Buy for Their Reasons

Another critical insight: patients don’t buy dentistry for clinical reasons—they buy for emotional ones.

That’s why the discovery process starts before the exam, uncovering each patient’s personal “why”:

  • A job interview
  • A wedding
  • A fear of ending up like a parent or grandparent who lost their teeth

When emotional motivation is connected to clinical findings, acceptance becomes natural—not forced.


The Case Presentation Formula

Only after patients understand the condition and consequences does the conversation move into solutions. Eric shared the structure behind All-Star’s proven case presentation formula:

  1. What’s important to the patient
  2. What the doctor is concerned about
  3. Why that concern matters
  4. How the patient feels about it

If concern isn’t present, treatment isn’t discussed. Trust is never violated. Pressure is never applied.

When communication is done well, the patient often asks the question themselves:

“What do we need to do to fix this?”

That’s true case acceptance.
